Fringe Benefits

The Lawndale Elementary School District provides fringe benefits, including medical, dental, vision, and life insurance to eligible employees. For enrollment forms and explanation of benefits, click the plan name.

Employees will have the opportunity to make changes to their coverage during open enrollment. Open enrollment begins late October through mid-November of each year. Changes are effective January 1st. 

Each plan also has additional resources available to subscribers.

There have been changes announced for MDLive and Kaiser Telemedicine due to the COVID-19 outbreak. Attached are flyers that announce the changes and below a few reminders from the carriers:

·         SISC MDLive - $0 Copays

o   In light of the COVID-19 situation, effective March 13, 2020, SISC is implementing $0 copays on MDLive services.  This benefit provides 24/7 access to a physician by phone.  MDLive physicians follow CDC guidelines with respect to persons exhibiting COVID-19 symptoms.

o   Anthem Blue Cross and Blue Shield Members - $0 flu shots are available at participating pharmacies. Members may call Navitus at 1-866-333-2757 for more information.

To make certain we can continue to provide the high-quality care our members need while also preparing for the oncoming increase in the number of patients with COVID-19 the following decisions have been made:

With gym closures and physical distancing, it can be a challenge for your members to stay physically and mentally healthy right now. That’s why we teamed up with fitness industry leader ClassPass to make it easier for your employees to exercise from the comfort of their homes.

With ClassPass, Kaiser Permanente members can get:

• Online video workouts at no cost — 4,000+ on-demand fitness classes, including cardio, dance, meditation, and more.

• Discounts on livestream fitness classes — Real-time online classes, like bootcamp, yoga, and Pilates, from top gyms and fitness studios.

Medical Plans

LESD offers two HMOs and one PPO.

Health Maintenance Organization (HMO):

  • United Health Care (800-624-8822)

Preferred Provider Organization (PPO):

  • Anthem Blue Cross - SISC (800-825-5541)
  • SISC has partnered with Health Design Plus (HDP) to offer their Oncology Center of Excellence Program to help covered members navigate their cancer diagnosis and treatment journey.  Please see below under Anthem Blue Cross for more details.

Waiving coverage?
Employees may waive coverage for a stipend (cash-in-lieu) if they are covered under a spouse or registered domestic partner. You must provide proof of spousal/partner coverage to your HR representative, by submitting a document on letterhead from the spouse/partner's employer stating current coverage dates and the health benefit plans you are covered under as a dependent. To waive your coverage, please complete the online form here.

Dental Plans

The district offers two dental plans, an HMO and a PPO. 

Health Maintenance Organization (HMO):

Vision Plan

The district offers one vision plan.

Life Insurance

The district provides life insurance and accidental death and dismemberment insurance through The Hartford. Additional/supplemental insurance can be purchased by the employee.

Retirement

The district participates in and contributes to CalPERS, CalSTRS, Metlife (Social Security Alternative), depending on your employment type. Typically, certificated employees are members of CalSTRS, full-time classified employees are CalPERS, and part-time classified employees are Metlife.

    • District contributes 16.28% (2018-19), 18.15% (2019-20), and 19.10% (2020-21). Employee contribute 10.205% (PEPRA) or 10.25% (Classic)
